Biceps Load Test - SLAP Special Test - Shoulder Clinical Examination 🧠 Want to learn more from me? Head to my profile and click the "Learn More" button to see all my latest articles and podcasts. The Biceps Load Test is special test that I will commonly perform as part of my cluster of tests for SLAP tears of the shoulder. I use this test a lot because it is more useful with overhead athletes and peel-back SLAP lesions. There are so many different types of SLAP tears, so you need to pick your s

Anterior Slide SLAP Special Test - Shoulder Clinical Exam 🧠 Want to learn more from me? Head to my profile and click the "Learn More" button to see all my latest articles and podcasts. One of the earlier tests described to detect a SLAP tear was the Anterior Slide Test. The test combines a compression and even a little anterior shear based on the alignment of the humerus. Unless the person may have had a compressive or traction type SLAP injury, I don't often see this test positive. It just doe

Clunk Test - SLAP Special Test - Shoulder Clinical Examination 🧠 Want to learn more from me? Head to my profile and click the "Learn More" button to see all my latest articles and podcasts. The clunk test is often performed during the shoulder clinical exam to diagnosis a SLAP tear. Remember, SLAPs can occur for various reasons, but a Clunk Test is fairly general enough to be useful for most. I often related the Clunk Test to some of our meniscus tests in the knee. We are essentially looking fo

Neer Sign – Shoulder Impingement Special Test 🧠 Want to learn more from me? Head to my profile and click the "Learn More" button to see all my latest articles and podcasts. The Neer Sign is one of the classic special tests for shoulder impingement. The examiner stabilizes the scapula while passively elevating the arm. Several studies have looked at this test and found it to not be very specific, but I feel it is still useful to detect overall inflammation and symptoms of the shoulder. 👊 Was th

Shoulder Shrug Sign – Rotator Cuff Special Test 🧠 Want to learn more from me? Head to my profile and click the "Learn More" button to see all my latest articles and podcasts. The shoulder shrug sign is often seen in those with rotator cuff tears. When trying to actively elevate the arm, the person is unable and often shrugs with the scapula and upper trap as the humeral head migrates superiorly. One way I like to examine a rotator cuff tear is to then actively elevate the person overhead and se

Speed's Test - SLAP Special Tests 🧠 Want to learn more from me? Head to my profile and click the "Learn More" button to see all my latest articles and podcasts. The Speed's Test was originally designed to detect biceps pain, however, many have often also used to during their shoulder clinical examination as a SLAP special test. I'm not 100% sure how specific the Speed's Test is for a SLAP tear. It never seemed to produce a positive test in my hands for SLAP lesions. I will perform it at times,

Sulcus Sign Here's a great example of a test for congenital laxity of the shoulder, the Sulcus Sign. Pretty simple to perform. I'll also use my finger to measure the gap and get a sense of the amount of laxity. You can perform this on the other arm for someone postoperative to get a general sense of their mobility. Meniscus Special Tests - Joint Line Palpation 🧠 Want to learn more from me? Head to my profile and click the "Learn More" button to see all my latest articles and podcasts. One of the most simply, yet important, special tests to detect meniscus injuries is joint line palpation. Many of the more specific special tests are very provocative in nature. Joint line palpation has been shown to have 0.95 reliability and varying fairly high amounts of sensitivity and specificity in numerous studies. Thi

UCL Special Test - The Milking Sign UCL injuries (Tommy John) continue to rise in baseball pitchers. The milking sign is a common UCL test that doctors love to do seated, essentially taking their arm back into layback and max external rotation. I just do this in the supine position and look for pain over the UCL. Quick. Simple. Effective. Do you want to become an expert working with baseball players? Check out my online course and become a Baseball Rehabilitation Specialist: https://mikereinold.

Posterior Drawer PCL Special Test - Knee Clinical Examination 🧠 Want to learn more from me? Head to my profile and click the "Learn More" button to see all my latest articles and podcasts. Still one of the classics, the posterior drawer test for PCL injuries is a gold standard. Remember muscle guarding in the acute setting can often limit this test, and be sure to assess both the amount of motion and the end feel. 👊 Was this helpful? Head to my profile and click the "Learn More" button to see all my latest articles and podcasts. The Apley Compression Rotation test is performed for meniscal tears of the knee. Originally, the distraction portion of the exam was thought to potentially test a ligamentous rotary instability, and the compression portion more for the meniscus.
